Neonatal Endotracheal Intubation Simple Guide

Step 1: Adjust the operation height. Put the newborn flat on a bed, so that its head and the operator's upper abdomen or lower chest at the same level.

Step 2: Stabilize the neonate's head in the "nasoaspiratory position": the neck is gently tilted to provide the best view of the airway.

(lying down head in a neutral position, with a 2cm shoulder pad or with the neck gently tilted).

We have designed our infant intubation trainer to do this successfully without requiring the practitioner to manually adjust the infant's head.

Step 3: Insertion of the laryngoscope

The laryngoscope lens should be slid in along the right side of the tongue, pushing the tongue to the left side of the mouth and advancing the lens until its tip is just above the root of the tongue to the epiglottic cartilage valley. Lift the entire lens, not just the tip. Expose the vocal folds. Firm downward pressure on the cricoid cartilage helps to visualize the vocal folds.

Step 4: Insertion of the tracheal tube

Enter the tube along the right side of the mouth and insert the tip of the catheter through the vocal folds until the midpoint of the trachea is positioned.

Step 5: Withdraw the laryngoscope

Step 6: Withdraw the metal tube core from the tracheal tube.

Step 7: Connect the resuscitation gasbag and perform frontal ventilation.

 

How to check if the intubation is correct and if the depth of intubation is appropriate?

In clinical:

(1) Heart rate and oxygen saturation improve

(2) Breath sounds are present in both lungs, but there is little or no sound in the gastric area

(3) Auscultation with a small stethoscope and placement on the lateral upper chest wall (axilla)

(4) The stomach does not dilate during positive pressure ventilation

(5) Symmetrical expansion of the chest with each breath

Exercise: the pink balloons representing both lungs are inflated while the red balloon is not.
