Buttock Pressure Ulcers Four Stages:
- Stage II: Partial-thickness skin loss presenting as a shallow open ulcer with exposed dermis.
- Stage III: Full-thickness skin loss with clearly defined wound edges.
- Stage IV: Full-thickness skin and tissue loss with exposed muscle or bone, often accompanied by necrotic tissue.
- Unstageable: Ulcer covered by slough or eschar, obscuring the wound bed and making accurate staging difficult.
